Pre- K and Kindergarten Immunization and Health Enrollment Requirements 2022-23
As your child enters Pre-K and Kindergarten it is more important than ever to protect your child against certain diseases. The most effective way to do this is to keep your child up to date on their required immunizations. The following is a list of required and recommended immunizations for kindergarten entry. Please provide immunization paperwork to the school nurse or office personnel prior to the first day of school on August 17, 2022. In addition, the CHIRP Consent Form and Parent Consent to Share Medical Information forms are included. The CHIRP Consent Form must be signed in order to allow the school nurse to keep your child’s immunization record as current as possible in the state registry. This ensures that parents and students will have access to immunization information when it might be needed in the future. For example, when required for college, military or job applications. The Parent Consent to Share Medical Information Form allows the school nurse to share medical information with other school personnel that need to be informed. Minimum Immunization Requirements Pre-K:
- 4 doses of DTaP
- 3 doses Polio
- 3 doses Hepatitis B (3rd dose of Hep B must be after the age of 24 weeks)
- 1 dose MMR (on or after 1st birthday)
- 1 dose Varicella (Chickenpox) (on or after 1st birthday)
- 2 doses Hepatitis A (the minimum interval between doses is 6 months)
Recommended Immunizations:
- Annual Influenza
- COVID-19
Minimum Immunization Requirements Kindergarten:
- 5 doses of DTaP/DTP/DT (4 doses are acceptable if the fourth dose was administered on or after the fourth birthday)
- 4 doses of Polio (3 doses are acceptable if the third dose was on or after the 4th birthday and at least 6 months after the previous dose and only one type of vaccine was used. The final dose must be administered on or after the 4th birthday and at least 6 months after the previous dose!)
- 3 doses of Hepatitis B (the 3rd dose of Hep B must be after the age of 24 weeks)
- 2 doses of MMR (on or after the first birthday)
- 2 doses of Varicella (chickenpox)-(on or after the first birthday and separated by 3 months or written physician documentation of the month and year of disease)
- 2 doses of Hepatitis A (The minimum interval between the dose #1 and dose #2 is 6 calendar months)
Recommended Immunizations:
- Annual influenza
- COVID-19
Immunizations can be obtained from your child’s health care provider or the St. Joseph County Health Department. If more resources are needed please contact your school nurse.
In addition, SBCSC requires proof of your child’s most recent Lead Screening Results.
